Contact LifeSpan Resources
Apply for the A&D waiver by visiting your Area Agency on Aging’s website for information on their referral process or by contacting us.
Telephone Assessment
Once your referral is received, your Area Agency on Aging will contact you for a phone assessment where they will ask you questions to understand your financial eligibility, medical information and ability to care for oneself.
Face-To-Face Interview
If determined eligible, your Area Agency on Aging will schedule an in-home assessment within 10 days. During this home visit, your Area Agency on Aging will ask you more in-depth questions about your needs and submit them to Indiana’s Division of Aging for services. Approval typically takes 20 days.
Service Plan
Once the intake process is complete, your Area Agency on Aging will provide ongoing case management. Your case manager will help you find the right support service or community setting that is right for you.
